Your Commitment
To keep hormone therapy safe and effective, Continuity members agree to:
Stay up-to-date on required labs and annual visits. (Failure to complete required testing may pause prescriptions.)
Maintain routine health screenings (mammogram, Pap, or other screenings as recommended).
Keep a primary care provider for non-hormone-related medical needs and urgent care.
Understand that hormone therapy involves both benefits and risks; results vary by individual.
