Mo' Glow Loyalty Membership Agreement

This Membership Agreement ("Agreement") is entered into by and between Soul Beauty LLC, DBA Mo Beauty Aesthetics and Wellness and/or Vitality Collective ("Provider"), and the undersigned individual ("Member").

1. Membership Details:

• Membership Level: Mo' Glow Loyalty Program

• Setup Fee: $49, one-time charge

• Monthly Cost: $149/month ($125 is deposited into your Mo’ Glow e-wallet to use toward services, the remaining portion of your membership supports built-in member benefits)

• Carry Over E-Wallet Dollars: Yes, up to 5 months.

• Initial Term: Minimum 6-month commitment from the date of signing.

• Renewal Term: Automatically renews monthly, unless terminated as outlined below.

2. Membership Benefits:

You are entitled to the following benefits as a Mo' Glow Loyalty Member. Discounts are applied to our published rates. A copy of published rates can be requested via email or when you are in the shop. Changes to our published rates can be made at the provider’s discretion with a 30-day written notice to members. 

• 10% off products and select services, including skincare, supplements, injectables, lasers, facials, microneedling, and medical weight loss

• 20% off IV Wellness drips

• Complimentary monthly use of the zero gravity massage chair and/or sauna (sauna is only in Dracut location)

• Priority booking access

• Exclusive access to new treatments and devices with special introductory pricing

• An upgraded annual Skin Strategy Session with a personalized 6–12 month plan

• Member-only wellness events and curated model call opportunities

• Birthday month perk: $25 credit toward skincare or services

• Referral appreciation: $25 wallet credit when a referred friend joins

4. Payment Terms:

• Membership fees are charged monthly using the payment method on file.

• An active credit card must be kept on file at all times.

• Late payments may result in a suspension or termination of membership benefits.

5. Scheduling and Cancellations:

• Unused E-Wallet Credits may roll over up to 5 months.

• Cancellations to your membership must be made, in writing, 30 days or more before the next billing cycle to avoid being charged for the subsequent quarter. Send emails to support@myvitalitycollective.com.

• If a Member cancels before the end of the initial 6-month term, they will be charged an early termination fee of $99.

• Appointment cancellations or rescheduling require a 24-hour notice. Failure to provide adequate notice may result in forfeiture of the scheduled service and appropriated E-Wallet dollars.

6. Membership Termination:

• Members may terminate their membership after the initial 4-month term by providing a written notice before the next monthly billing cycle. Send emails to support@myvitalitycollective.com.

• The provider reserves the right to terminate the membership if the terms of this agreement are not met.

7. Miscellaneous:

• Loyalty membership benefits are non-transferable and may not be shared.

• Provider reserves the right to modify benefits, services, or pricing with 30 days' prior written notice. Changes will be posted online, via email, or within the Aesthetic Record Membership Portal.

• Members agree to use the Aesthetics Record Membership Portal to manage their accounts and book future appointments.
